Documents required for

Social Security (ZUS) / Agricultural Social Insurance Fund (KRUS)  / 
The Ministry of Internal Affairs and Administration (MSWiA) /
Ministry of National Defence (MON)

- for payment of the funeral grant

  • Death certificate - the original issued from the Registry Office (without stamp duty).

  • Certificate from the priest regarding the burial ceremony.

  • Certificate from the cemetery administrator regarding the burial site.

  • A pensioner card (of the deceased person)  or other document confirming insurance, e.g .:

    • the old-age / disability pension claim or the decision on granting the old-age / disability pension

    • a certificate from the workplace (of the deceased person) on employment and social security contributions paid until the day of death (the certificate should include the PESEL number of the deceased) or another document confirming the insurance of the deceased person.

  • If the deceased person did not have insurance - then a valid document confirming the insurance of the person entitled to the burial (pensioner ID or certificate from the workplace on employment and social contributions)

  • Birth certificate / marriage certificate of the person ordering the burial - a document required to establish the family relationship with the deceased (the certificate does not have to be up-to-date, i.e. issued for the last three months). The document is not required if the burial is organized by the spouse of the deceased.

If the act is collected at the Registry Office, please inform that it is necessary for submission to ZUS - then it will be issued on application without stamp duty.

  • ID card of the person ordering the funeral ceremony.

At the request of the entitled person, the original documents can be collected from the Social Insurance Institution (ZUS) after considering the application for a funeral grant.
