Proceedings in case of death

DEATH AT HOME

If the death occurs at home you should call a doctor, who will determine the death on the basis of visual inspection and issue a death card or information card . If they issue an information card, it should be taken to the general practitioner (at the clinic where the deceased had a treatment card) - then a death card is issued on the basis of the information card. The death certificate is a document necessary to issue a death certificate.

After the doctor confirms the death, you need to contact the selected funeral home to transport the body to the cold store.

With the death certificate and ID card of the deceased, you should go to the Registry Office nearest to the place of death
and collect the death certificate . Depending on the internal regulations of a given office, more than one copy of the document may be issued without stamp duty. The Registry Office in Końskie issues 1 free copy , which will be submitted to ZUS (as an attachment to the application for funeral grant), therefore we advise you to collect additional copies, which may be useful when completing formalities in banks, workplaces, insurance companies, etc.

The next step is to arrange the date of the funeral with the priest and the administrator of the cemetery . If the funeral is to be held at the cemetery in Końskie, we recommend that you first go to the gravediggers' office at  Browarna St., and then to a priest from the parish of the deceased. Both the priest and the gravedigger will ask for the original of the death certificate.

DEATH IN A HOSPITAL

When the death occurs in a hospital, the doctor draws up a death card and the body is transported to the dissecting room. The doctor may order an autopsy - if the family does not agree and there are no contraindications for withdrawal, a written request should be made to the director of the hospital. The death card should be collected from the hospital (currently, due to the pandemic, cards are not issued in wards - you have to go to the administration).

DEATH AS A RESULT OF TRAGIC CIRCUMSTANCES

If the cause of death is an accident, suicide or there is a suspicion of murder, first of all you need to notify the police and the ambulance service. After confirming the death, the case is taken over by the public prosecutor. Most often, in such circumstances, the prosecutor orders a post-mortem examination. If he finds that the death did not involve third parties, he may refrain from ordering the autopsy.

The HADES Funeral Home has an agreement with the District Prosecutor's Office in Kielce for the transport of deceased persons for whom an autopsy is to be performed . Pursuant to the aforementioned agreement, the autopsy is done in our autopsy room and we are authorized to give a death card (issued by a pathologist) to the family .

After the autopsy is over, the consent for burial should be obtained from the prosecutor's office. Without this document, it will not be possible to collect the body from the dissecting room.
